We report an unusual case of an advanced stage of CD5 negative MCL with a blastoid variant with a massive bone marrow (BM) necrosis as an initial presenting feature Mantle cell lymphoma (MCL) characteristically express CD20, CD5, and cyclin-D1, carries the translocation t(11;14) (q13;q32) and typically has no expression of germinal center cell markers. Mantle cell lymphoma (MCL) is an uncommon type of B-cell lymphoma that represents 3–10 percent of the non-Hodgkin mature B-cell lymphomas . MCL predominantly occurs in older patients with a higher frequency in men, usually presenting with widespread disease, and has an aggressive clinical course.
